Fill out the following form if you have an incoming kindergartener or if you are enrolling a new student.

1. Student Demographic Form: Complete, Print and Sign

4. Proof of Residency: Students must enroll in the school within the boundary where they reside.  Please refer to the Proof of Residency form for accepted documentation. Complete, Print and Sign

6. Create a myDSD account
